We are concerned about your dental health and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment procedure.
Payment is due for treatment at the time of service. For your convenience, payments may be made with cash, personal check, and bank draft, Discover, Visa, or MasterCard. If you have any problems or questions, please ask our staff. They are well informed and up-to-date. They can be reached by phone at 216-731-7555 or by fax at 216-731-5503. Our office location is Euclid Medical Plaza 26300 Euclid Avenue Suite 424 Euclid, OH 44132. Please don’t hesitate to call if you have any questions or concerns regarding your initial visit.
Dental insurance often provide benefits to be used toward your necessary periodontal treatment. Some employers offer many different dental and medical plans. Your employer can change benefits, co-pays, and deductibles many times throughout the year. We do our best to provide you with accurate coverage estimates based on the information that is available to us. At times, it is almost impossible to accurately estimate a patient’s insurance benefit. Many insurance companies will not give out fees until after the treatment is completed. We are happy to assist you in maximizing your insurance benefit for your necessary treatment. Our patients are informed that dental benefits do not dictate treatment that is necessary and needed for your oral and systemic health. Dental benefits are an arrangement with the employer and the patient as a benefit to assist with treatment fees, not to cover entire procedures or to determine if treatment is needed or necessary. Dr. Woznicki’s trained staff will be happy to answer any of your questions.
We will gladly file a claim on your behalf at the time of service. Most dental insurance benefits are very limited and often only pay for a portion of the procedure(s) that may be necessary for your oral and systemic health. Any insurance benefit will be sent directly to the patient.
